Introduction

Telehealth involves the use of electronic communications and information technology to enable licensed healthcare providers to evaluate, diagnose, and treat patients when the provider and patient are not in the same physical location.

This Telehealth Informed Consent explains the nature, benefits, risks, and limitations of telehealth services made available through the Harbor platform and describes your rights as a patient.

Platform Role and Medical Care

Harbor is a technology platform operated by SC Studios Inc. Harbor does not practice medicine, provide medical care, or make clinical decisions.

Medical care is provided solely by independent, licensed healthcare providers within Harbor's provider network. These providers establish and maintain the provider–patient relationship and are solely responsible for all clinical decision-making, diagnoses, and treatment recommendations.

Scope of Services

Telehealth services accessed through Harbor are limited to weight loss and metabolic health care.

Services may include clinical evaluations, treatment recommendations, and, where medically appropriate, prescriptions. Harbor does not provide mental health services, emergency care, or crisis services.

Eligibility

Telehealth services are available only to individuals who are at least eighteen (18) years of age and physically located within the United States at the time of the consultation.

Modality of Care

Telehealth services may be delivered through asynchronous communications, such as secure messaging, and/or synchronous communications, such as audio or video consultations, depending on clinical appropriateness and applicable state law.

Not all services are available in all states.

Frequency of Care

Clinical consultations typically occur on a periodic basis, including approximately every three (3) months, as determined by your provider.

If prescribed, medications may be provided on a recurring basis, subject to provider oversight, medical necessity, and applicable law.

Limitations of Telehealth

Telehealth has limitations compared to in-person care. Providers are unable to perform physical examinations, and some medical conditions may not be appropriate for telehealth.

Your provider alone determines whether telehealth is appropriate for your care. There is no guarantee that telehealth services will meet your medical needs or result in a prescription.

No Emergency Care

Telehealth services accessed through Harbor are not intended for medical emergencies.

If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not attempt to use Harbor or contact a provider through the platform for emergency care.

Risks of Telehealth

Risks associated with telehealth may include delays in care due to technical issues, limitations in clinical assessment, and risks related to the electronic transmission of information.

While reasonable safeguards are used to protect your information, no electronic system can be guaranteed to be completely secure.

Prescriptions and Pharmacies

There is no guarantee that a provider will issue a prescription. Prescriptions are issued solely at the provider's professional discretion.

If a prescription is issued, you have the right to choose the pharmacy of your choice. Harbor is not a pharmacy and does not dispense medications.

Privacy and Confidentiality

Your health information is protected under applicable federal and state laws.

Medical records created as part of your care are maintained by the provider or provider network. Harbor's collection and use of personal information is governed by its Privacy Policy.

Communications

By using the platform, you consent to receive communications related to your care and use of the services through secure platform messaging, email, text message, or phone, as permitted by law.

You may opt out of non-essential communications at any time. Consent to receive communications is not a condition of receiving medical care.

Right to Withdraw Consent

You may withdraw your consent to receive telehealth services at any time. Withdrawing consent will not affect your right to seek future care outside of the Harbor platform.

Acknowledgment and Consent

By proceeding, you acknowledge that you have read and understand this Telehealth Informed Consent, understand the benefits, risks, and limitations of telehealth, and voluntarily consent to receive telehealth services through the Harbor platform under the terms described above.
