Payment & Billing Consent

Introduction
By clicking where indicated, you agree to this Payment & Billing Consent ("Agreement"). This Agreement explains your financial responsibilities when using the Harbor platform and supplements Harbor's Terms & Conditions, which are incorporated by reference.
Harbor is a self-pay telehealth platform. We do not currently accept health insurance, though insurance acceptance may be offered in the future. We do accept HSA and FSA payments where permitted.
Self-Pay Services
At this time, all services offered through Harbor are provided on a self-pay basis. You are responsible for all charges associated with your subscription and any services or products provided through the platform.
Harbor does not submit claims to health insurance plans on your behalf and makes no representations regarding insurance reimbursement.
Subscription Plans and Billing
Harbor offers subscription plans that may be billed on a monthly or quarterly basis, depending on the plan selected at checkout.
Subscriptions automatically renew unless canceled. There is no minimum commitment, and you may cancel your subscription at any time. Cancellation stops all future charges. Access to services may continue through the end of the current billing period, unless otherwise stated at checkout.
Subscription pricing, plan features, and offers may vary by plan, promotion, or availability. Harbor reserves the right to change pricing or plan terms with at least thirty (30) days' prior notice, where required by law.
What the Subscription Includes
Your subscription generally includes access to the Harbor platform, ongoing provider oversight, periodic clinical consultations (typically approximately every three months), and medication fulfillment and shipping if prescribed.
There is no guarantee that a provider will prescribe medication. Medication availability and cost may vary based on clinical appropriateness, supply, and other factors.
Payments and Failed Transactions
By enrolling, you authorize Harbor to charge your selected payment method for all applicable subscription charges and fees.
If a payment attempt fails, Harbor may reattempt payment up to three (3) times. If payment cannot be successfully processed, your subscription may be paused until payment is resolved.
You are responsible for maintaining accurate and up-to-date payment information.
HSA and FSA Payments
You may choose to use HSA or FSA funds to pay for eligible services. It is your responsibility to determine whether your subscription or services qualify for HSA or FSA reimbursement under applicable rules.
Refunds and Cancellations
Charges for services or products that have already been fulfilled, including prescribed medications that have been prepared or shipped, are non-refundable except as required by law.
If treatment becomes unavailable prior to fulfillment, Harbor may issue a prorated refund where appropriate.
Disputes and Chargebacks
If you have a billing concern or dispute, you agree to contact Harbor's support team before initiating a chargeback with your financial institution.
Billing disputes are governed by the dispute resolution and arbitration provisions set forth in Harbor's Terms & Conditions.
Agreement
By agreeing to this Payment & Billing Consent, you acknowledge that you understand your financial responsibilities, authorize Harbor to charge your payment method as described, and agree to the terms of this Agreement and the Terms & Conditions.
Your electronic acceptance of this Agreement is valid and binding.
